Office Policies

Welcome to Gastro Health! As you prepare for your appointment, please:

  • Bring your driver’s license, insurance cards (primary and secondary), and your co-pay.
  • Review your insurance coverage. It is important for you to understand the extent of your health insurance coverage and your financial responsibility.

  • Your insurance may be required to obtain a referral to be seen by one of our physicians. Please contact your member services to determine if a referral is required. Referrals must be in our office before or at the time of your appointment. Patients can bring their referral or have their referring physician fax it to our office. Please note, most primary care offices require at least 48 hours’ notice to prepare a referral.

  • Please refer to the phone number on your billing statement. If you do not have a bill yet, please email questions to info@gastrohealth.com.

  • We encourage you to review your insurance coverage prior to your appointment or procedure. Most insurance plans have co-pays associated with coverage. We are required to collect these payments at the time care is rendered.

  • Appointments are scheduled so that each patient receives the right amount of time to be seen by our physicians and team. That’s why it’s very important that you keep your scheduled appointment with us and arrive on time.

Patients who cancel or reschedule an appointment with less than 24 hours’ notice will be assessed a $50 fee.

*For Monday appointments, cancellations/rescheduling must be done by 12 noon the preceding Friday.
