Office Policies
New Patients
To follow are a few things that you, as a new patient, will need to do prior to or at the time you first visit our office:
- Understand the extent of your insurance coverage for office visits and any procedures that you are anticipating in the course of your care with our physicians.
- Review our Insurance and Billing policies and Notice of Privacy Practices statement.
- Complete our New Patient Forms and bring them with you to the office.
- Bring your drivers license, your primary or secondary insurance cards and your copay with you.
Returning Patients
Each time you return to our office or are scheduled for a procedure at our endoscopy center or infusion suite, we will need to verify your insurance benefits. Please remember to bring your insurance cards and your driver’s license since it will help make the registration process easier for you. We also ask that you inform us of any changes to your home address or telephone numbers.
Office Hours
Our office is open between the hours of 8:30 a.m. and 4:30 p.m. Monday through Friday (excluding holidays) and closed on the weekends
Prescription Refills
The most convenient way to refill a prescription is for you to contact your pharmacy directly. In most cases, the pharmacist will be able to refill your medication using the information contained on the label of your prescription bottle. Even if you have no more refills, your pharmacy can contact our office to renew your prescription. If additional information is needed, someone from our office will contact you to discuss your need for additional medication or arrange an office visit to ensure proper care.
We request that you provide us with 48 hours notice so that the secretary will have a chance to contact your doctor and the pharmacy. If prefer, you can request a refill by contacting the office where you routinely receive care. Prescriptions for narcotics and pain medications will be filled only during office hours, Monday through Friday, and not after hours or on weekends.
Emergencies
If you have a medical emergency, please seek emergency assistance by dialing 911. Outside of our normal business hours, one of our physicians is on-call 24 hours a day, 7 days a week. The on-call physician can be reached by calling the office where you are routinely seen. An operator will take your call and forward your message to the on-call physician who will later contact you.
