Upon scheduling an appointment, you will receive an email to confirm your appointment and securely submit your registration information electronically. The email will come from Digestive Disease Associates <email@example.com>. Please check your SPAM and Junk folders if you don’t see the email in your inbox. If you don’t receive the email, please call the office.
If you are unable to complete your registration electronically prior to your visit, please arrive at least 30 minutes prior to your appointment to utilize our tablet check in system.
Please remember to bring your insurance card, copay and referral if applicable.
Appointment and Procedure Cancellation and No Show Policy

Patients who cancel or reschedule an appointment with less than 24 hours notice will be assessed a fee of $50.
*For Monday appointments, cancellations/rescheduling must be done by 12pm the preceding Friday.
Patients who cancel or reschedule a procedure with less than 48 hour notice will be assessed a fee of $100.
*For Monday procedures, cancellations/rescheduling must be done by 12pm the preceding Thursday.
If you are unable to call the office, you may use the form under Contact Us to cancel or reschedule your appointment or procedure. You must still adhere to the time frames above.
